Role Overview:
Oldham Council are in the process of transforming Community Mental Health Services and are having specialist Social Work Teams working with the people of Oldham who have severe and enduring mental ill health. You will be co-located with Health colleagues working towards providing a comprehensive service to the people on your caseload. You will be undertaking Care Act assessments and supporting your clients will be as independent as possible.
Key Responsibilities:
* To work in partnership with your clients and their carers to ensure their wellbeing and promote good mental health.
* Undertake Care Act Assessments, creating packages of care that will meet the needs of your clients and review them on a regular basis to ensure they continue to meet their needs.
* To safeguard the needs of your clients whenever needed.
* To record case notes, assessments & reviews to ensure they are clear and contemporaneous.
Key Requirements:
To succeed in this role, you will need as a minimum:
* A Social Work Degree or equivalent qualification.
* Experience of working with people with a mental health disorder.
* Experience of working in a community team as a Social Worker.
* Excellent communication and interpersonal skills to work with people with severe & enduring mental ill health and their carers.
