About the role:
We are looking for an experienced Adult Social Worker to facilitate hospital discharges, working in a timely manner and ensuring reviews are undertaken within 2 weeks aligning with a winter initiative process.
RIACT are looking for experienced Social Workers to support in the progression of the winter initiative whereby we ensure a home first approach. For those unable to return home at point of discharge to ensure regular reviews to consider home at the earliest safest opportunity.
They would prefer individuals who are able to work 5 days per week, although this may be negotiable dependent on the candidate. Additionally with an expectation of 3 days per week office based therefore living locally is cruicial. Immediate starts available.
What you need:
5. A degree in Social Work or equivalent
6. Registered with Social Work England
7. Completed relevant post-qualifying training
8. Enhanced DBS check
9. Excellent communication and written skills
