East Lothian Hospital to Home Team - Healthcare Support Worker Opportunities
The East Lothian Hospital to Home team has exciting opportunities for Health Care Support Workers to join their new dedicated team.
We are creating a team within our Hospital to Home service, which collaborates with local health and social care partners, including NHS Primary Care, East Lothian Council, and voluntary organizations, to support patients in staying longer at home.
Your role will involve assessing, planning, providing, managing, monitoring, and evaluating nursing care in a community setting. Responsibilities include assisting with activities of daily living, medication management, meal preparation, and providing end-of-life and palliative care.
What are we looking for?
* Qualifications: Basic literacy skills, good education, and English language competency necessary for the role.
* Personal qualities: Good health, reliability, strong observation skills, initiative, teamwork, enthusiasm for elderly care, supportive references, and physical ability to perform duties.
* Note: A full driving license is essential, and the team provides a fleet of cars for staff use.
