An exciting opportunity has arisen to join the Families' Health and Wellbeing 0-19 Service delivering public health services to children and families across Staffordshire. We have a vacancy offering 22.5 hours a week for a Band 6 Public Health Practitioner (HV) based in the Burton 0-19 Universal team, which has recently been awarded the Maximising Experience at Work Excellence Team of the Year award.
The Families' Health and Wellbeing Service (0-19) in Staffordshire offers an integrated, flexible, and family-focused service providing high-quality, compassionate, personalized support and care to Children and Young People across Staffordshire. You will be part of a team delivering the 0-19 Healthy Child Programme for a diverse population, working closely with partner agencies across health, social care, and the local authority.
The successful applicant will need to adhere to the service specification and demonstrate the trust values at all times. Knowledge and understanding of safeguarding children are essential. Responsibilities:
* Be competent as appropriate to the requirements of the role.
* Work within your scope of practice.
* Demonstrate the Values of Midlands Partnership Foundation.
* Be a Specialist Community Public Health Practitioner with detailed professional and clinical knowledge, focusing on public health across the 0-19 agenda.
* Manage a defined population of service users across various settings within the Trust, including community environments.
* Assess children and young people's care/support needs and develop, implement, and evaluate care/support plans.
* Take on supervisory, training, and leadership responsibilities where appropriate, contributing to team performance and service improvements.
* Demonstrate initiative and creativity in problem-solving.
* Work with the team leader on delegated tasks.
* Deliver aspects of the Healthy Child Programme, ensuring key objectives and outcomes are met.
The successful applicant must be able to travel independently within the Trust.
