Role Overview
We are seeking to recruit an experienced and compassionate Community Mental Health Nurse to the role of Band 7 Team Leader within the Older Adults Community Mental Health Team.
This post provides clinical, professional and operational leadership to a community mental health nursing team delivering specialist care to older adults with functional mental illness, dementia and complex comorbid physical health needs.
Responsibilities
The successful candidate will support delivery of high quality assessment, risk management and person‑centred interventions within community settings. You will maintain a reduced caseload of complex service users, support staff wellbeing and development, and contribute to service improvement and clinical governance activity.
The role involves close partnership working with carers, primary care, social work, inpatient services and third sector partners, ensuring safe, effective and coordinated care across the Older Adults pathway.
Qualifications
Applicants must be Registered Mental Health Nurses with significant postregistration experience and evidence of leadership or supervisory experience within mental health services. Driver and access to a car essential to role.
Work Hours
Monday to Friday 36 hours per week.
