The Clinical Service Lead (Community Nursing Care) will support the wider leadership team within urgent care working alongside the Clinical Director, Deputy Clinical Director & Head of Palliative and Community Nursing in the delivery of Community Nursing Care services across Derbyshire. The post holder will provide professional clinical and operational leadership to an integrated team ensuring that processes are in place to support the achievement of organisational goals, a culture of patient safety and high standards of care delivery. The post holder will work blended hours during the week. This will include 30 in-hours and 7.5 clinical out-of-hours per week within the service. This will ensure a visible presence to support both clinical and operational teams. Role and Responsibilities: Provide visible, compassionate, and effective clinical leadership across the Palliative Care service, fostering a culture where roles, responsibilities, and professional accountability are clearly understood and staff are empowered to deliver high - quality care. Support the Senior Leadership Team to ensure services operate efficiently, meet contractual and commissioning requirements, and align with organisational priorities. Lead the ongoing review, development, and implementation of palliative care clinical pathways, ensuring compliance with quality standards, national guidance, and agreed key performance indicators. Provide expert clinical oversight and contribute to the development, implementation, and monitoring of clinical standards and best practice, using evidence - based decision - making and sound clinical judgement. Contribute to service development initiatives, ensuring agreed changes are implemented effectively and evaluated for impact and patient outcomes. Support delivery of CQUINs and other quality initiatives relevant to palliative and end - of - life care.
