An exciting opportunity has arisen within the Older Peoples Community Mental Health Team. The post holder will be responsible for contributing to the care and support of older people affected by mental health and their relatives referred to our service by their GP.
The post holder will mainly be involved in the assessment of patients at the memory clinic or assessing patients in their own home.
The post holder will have to undertake duties without direct supervision which includes working in partnership with patients and their relatives, liaising with third sector agencies, line managers and other staff within the Older Peoples Community Mental Health team and wider HSCP.
The post holder will be responsible for the management of the memory clinic which is based at the Belmont Centre on a Monday and Tuesday 9am to 5pm.
The post holder will engage in regular weekly MDT’s.
The post holder will participate in regular supervision both caseload management and clinical supervision as these are critical aspects of a Band 5 Community Psychiatric nurse.
The post holder should be registered with the NMC and have a driving licence as home visits may be necessary.
