Adult’s Social Worker, Frailty and Anticipatory Care Team, North London Pay rate – £32.52 per hour Contract role, Frailty and Anticipatory Care Team Pertemps are recruiting for experienced Adult’s Social Worker in North London. This is to work in a Frailty and Anticipatory Care Team Key Responsibilities Deliver strengths-based assessments and interventions under the Care Act, Mental Capacity Act, and other relevant legislation. Work collaboratively with residents, carers, community organisations, and health professionals. • Attend and contribute to multi-disciplinary team (MDT) meetings, Identify and respond to safeguarding concerns using a Making Safeguarding Personal approach. Maintain accurate records using the LAS electronic system, ensuring timely documentation of outcomes and progress. Support anticipatory care planning and early intervention to prevent escalation of need.
