Job summary
Working under thedirect supervision of the Nursing Manager and strictly in accordance withspecific practice guidelines and protocols; the Healthcare Assistant will primarilyassist the ANP Home Visit team in the support of houseboundpatients with long term conditions, supporting their reviews and ongoing care. Thisforms the core of the work stream, but the individual should ideally have theflexibility to be able to work within the practice setting if required.
The required working hours for this role will be 15-20 and will be discussed with the successful candidate.
Main duties of the job
1. Ensure the delivery of nursing care is evidencebased and own practice is clinically reviewed through supervision and reflectivepractice as part of revalidation.
2. Provide clinical support to the Long TermConditions team and promote collaborative working relationships between allmembers of the multi-disciplinary team.

Health & Safety:
