Job Summary
To assist registered nurses/practitioners to deliver care to patients/clients, working in partnership with other health care professionals, the patient their family and carers.
Main Duties
Deliver patient care as documented in the nursing care plan. Carry out a wide range of duties under the direction of a registered nurse for the period of the shift.
Support the district nursing team to care for those living in the community who require nursing care. Travel between patient homes both within towns and rural areas in a timely manner. Communicate with patients, families and carers in Welsh where necessary. Work well within a team and also independently.

* Band: 3
* Salary: £25,313 to £26,999 a year per annum, pro rata
* Contract: Permanent
* Working pattern: Full‑time
