Job Title: Physiotherapist Banding: Band 6 Location: Hounslow, Greater London Start Date: ASAP Duration: 3 months Rate: £30.00 per hour Working hours: Full-time We are seeking a highly skilled and motivated Band 6 Physiotherapist to join our esteemed client in Hounslow. You will work collaboratively within a multidisciplinary team and your caseload will consist of supporting triage referrals for both the Discharge Hub and Home First services. Key Responsibilities In this role, you will be responsible for triaging referrals via telephone, assessing patient needs, and coordinating appropriate care pathways. You will play a key role in facilitating timely discharges and supporting patients transitioning from hospital to home. Your clinical judgement and communication skills will be essential in ensuring safe and effective care planning. Qualifications and Experience: HCPC registration. Bachelor’s degree in Physiotherapy or a related field. Experience in community or discharge planning settings is desirable. Strong clinical reasoning and decision-making skills.
