Job summary
As a Care Transition Nurse, you will be working to support the discharge function; providing flexible, responsive, proactive and interactive assessment, care and management to individuals who require funding to enable discharge from acute in patient services. Ensuring empowerment of the service user and their carers / family throughout the process.
It is Trust policy that for posts that are for a time limited period, applicants who are currently employed by an NHS organisation will only be appointed on a secondment basis and should have approval from their line manager this is possible before applying. Applicants who are not employed by an NHS organisation, will receive a fixed term contract.
Main duties of the job
1. Establish the role within all Acute Inpatient Areas by attending weekly clinical meeting and appropriate ward meetings.
2. To identify present in-patients who would benefit from an alternative placement /or package of care to enable an early discharge from in patient care.
3. To work closely within the in patient service, community and other professionals involved with an individual's care.
4. To introduce placement and community package options to service users and carers/ family.
5. To support the clinical team to complete and update the required paperwork as part of funding applications. To ensure timescales are determined and adhered to. The post holder will also be expected to participate in the completion of the required documentation.
6. The post holder to submit completed documentation to the relevant agencies and will follow up on progress of application and offer regular updates to clinical team.
7. The post holder may be involved in some co-ordination of attendees of the care team to the appropriate funding panel.
8. The post holder may at times need to address conflict of opinion over appropriate placement/ packages with professionals, the clinical team and patients, carers and families.

The role will be part of the current bed management team, working alongside a full time Transition of Care Nurse based at the Radbourne Unit. The post holder will support patients leaving the ward and being discharged to community into other care provisions or to their home address.
Supporting patients to engage with local community based support networks to encourage and maintain independence within the local community when they no longer require an acute hospital bed. This will include working alongside social care and third sector providers to ensure the right place at the right time is achieved .
