Role: Resettlement Coordinator
Responsible to: Team Leader and Service Manager
Hours/Shift Pattern: 40 hours – 5 days per week (rota basis including some weekends)

Aims of the service:
Kickstart Housing provides accommodation to people with vulnerabilities. Our services include accommodation in purpose built houses for people experiencing homelessness. We work closely with our residents to build on their strengths and to form effective working relationships through our care, respect, empathy and commitment.
Our aims are to successfully accommodate those who are rough sleeping or at risk of homelessness, providing respite from rough sleeping and coordinating assessments into longer-term suitable accommodation; supporting Government strategy and Local Authority ambitions to end rough sleeping.
Key Roles and Responsibilities:
* Providing housing related support to individuals who are currently rough sleeping or at risk of homelessness.
* Provide pathways, support and appropriate interventions to enable successful move on into
community accommodation.
* Managing referrals for new service users via our partners within the community, providing assistance and information regarding the service delivery plan, suitability and the referral process.
* Work with internal and external partners, agencies and commissioners including, Rough Sleeping Outreach Teams and other community-based agencies.
* Key working a case load of service users from initial assessment, creating a personal outcome plan, reviewing the plan whilst working towards tangible move-on options.
* Supporting individuals in the community, alongside outreach duties involving support to build daily living and Independence skills i.e social inclusion, shopping, support groups
* Create robust risk assessments to manage the safeguarding factors of the individual.
* Partnership working with relevant professionals, agencies and providers to ensure that suitable support is available and provided for the service user.
* To maximise effective communication between all partnerships so that the desired outcomes are achieved for the individual.
* Helping increase the availability of private sector accommodation and the chances of successful move on into private sector tenancies
* Working in close partnership with partnership agencies, support providers, facilitate clear communication and information sharing in line with Kickstart Housing and legislative data protection and confidentiality safeguards.
* Recording all service user contact including updating resettlement plans, contact details and risks as necessary
* Supporting Service Users to attend appointments in the community, promoting social inclusion and supporting transition into further accommodation.
* Ensuring records are maintained and updated in line with internal and regulatory policies and procedures.
* Building positive partnerships with the local authority, service providers and agencies, signposting service users to relevant agencies to increase participation, health and well being, social inclusion, volunteering opportunities and pathways into paid work.
* Maintaining the health and safety, security of our properties as per the needs of the service, inclusive of managing voids, preparing rooms for new service users and maintaining communal spaces.
* Facilitating maintenance needs within the service alongside the property team as required, raising work orders, managing the service calendar and meeting contractors etc.
* Actively participate as a member of the team to maintain high standards and continuous improvement.
* Attending regular team meetings and briefings.
