An opportunity has arisen for a Care Coordinator to join our friendly team, coordinating for our Warwickshire & Coventry branches. You will be responsible for scheduling care hours, matching clients with Caregivers to ensure high-quality, person-centred care. Ideally, you will have your NVQ Level 3 in Health & Social Care or be working towards it.

Responsibilities
* Ensure all regulatory and contractual standards are met and exceeded.
* Provide leadership and management to a team of caregivers, ensuring skills and competencies are maintained, and promote continuous professional development.
* Assess current clients and plan and provide high-quality, person-centred care.
* Represent Unique Senior Care effectively and collaborate with local NHS, social services, and community contacts.
* Support the Registered Manager with writing care plans.
* Engage in business development and outreach in the community, building relationships with external stakeholders.
* Communicate effectively with caregivers, clients, families, and other parties.
* Manage caregiver schedules, including planning annual leave and time-off requests.
* Manage client schedules to meet care plan outcomes and requests.
* Match Caregivers and Clients to ensure appropriate support.
* Maintain accurate records on our management system, recording all discussions.
* Interview incoming caregivers.
* Ensure compliance with all legal and company requirements.
