Admiral Nurse

A friendly, professional Admiral Nurse will deliver a specialist service across Northumbria Primary Care (NPC) and Northumberland. This is a new and exciting collaboration between Dementia UK and NPC. Admiral Nurses are specialist dementia nurses who offer expert practical, clinical and emotional support to families involved in the care of a person living with dementia with complex needs.
We are looking for someone who is driven and committed to developing this new Admiral Nursing service, and who is passionate about helping those with Dementia.
The position will be of special interest to nurses looking to enhance their leadership and communication skills. We are looking for a skilled Registered Nurse either already working as an Admiral Nurse or from an adult/mental health/learning disability background.
You will have a passion for, and a demonstrable track record of, providing person-centred and relationship-centred dementia care. You will have the ability to combine direct work with patients and also their families, to support them with the best practice of health and social care professionals. You will also possess experience of delivering bio-psychosocial interventions to improve the experience and quality of life of families affected by dementia.

Main duties of the job
You will be involved in developing the service to improve the experience of carers/families affected by dementia with complex needs.
In line with the Admiral Nurse Competency framework at a specialist level, the post holder will provide:
* Specialist bio-psychosocial assessment and support for people with dementia and families with complex needs.
* Facilitation of education and training for staff in the delivery of evidence-based dementia care.
* Clinical leadership with policy development and quality improvements in relation to dementia care.
* Consultancy to other health and social care professionals regarding dementia.
You will work to develop this new role and service with NPC Head of Nursing, the Lead Admiral Nurse within Northumbria Healthcare NHS Foundation Trust and the Dementia and Delirium Steering Group.
The post holder will need to have the ability to work in a variety of settings, across a wide and sometimes rural geography in Northumberland. The role will include seeing carers and families individually, managing your workload, running support groups and providing education, as well as balancing a proxy-clinical caseload. This is also an exciting opportunity for the Admiral Nurse to create, develop, maintain and evaluate this exciting new service.

Northumbria Primary Care Ltd (NPC) is a pioneering partnership with local GPs and now manages several GP practices with around 120,000 registered patients in Northumberland and North Tyneside.
Whilst appointees will have a base practice within NPC, they may be asked to work at other practices across our group. We do however limit this as much as possible as we appreciate continuity is vital to effective primary care.

Job responsibilities
The Admiral Nurse will support, deliver and coordinate relationship-centred dementia care across NPC.
You will be based within a multi-disciplinary, primary care network, working alongside nurses, therapists, GPs, social care and third sector providers, amongst others. You will also benefit from being a part of a Clinical Network of Admiral Nurses working across England and Wales and will have the opportunity to access relevant post-graduate training and develop professionally, in line with the Admiral Nurse Competency Framework.
Dementia UK will support you to maintain and strengthen your clinical and leadership expertise as a specialist Admiral Nurse through regular clinical supervision and practice and professional development opportunities. You will be accountable to NPC Head of Nursing and the Lead Admiral Nurse within Northumbria Healthcare NHS Foundation Trust.
Person Specification
Qualifications / Professional Registration
* Registered Nurse (RMN/RN/RNLD) 1st Level Current NMC Registration
* Degree in Nursing or relevant degree level study
* Teaching or Mentorship Qualification
* Post registration training/ education in dementia/ frailty/ older people
* Post Graduate or Masters level qualification in dementia / older people's care/ frailty
Experience and knowledge
* Significant post registration clinical experience of working with people with dementia, and their cares/ families in different settings including acute care.
* Experience of completing holistic nursing assessments
* Experience of providing biopsychosocial interventions
* Experience of recent involvement in practice/ service development
* Experience of facilitating and delivering training
* Experience of working as an autonomous practitioner
* Experience of managing care across care transitions
* Experience of professional supervision of staff
* Experience of audit/ evaluating clinical practice
* Evidence of continued professional development
* Experience of undertaking staff performance reviews and planned development of staff
* Experience of working with patients/ clients to develop practice
* Experience of older persons medicine and palliative care
* Experience of supporting systematic practice development/ quality improvement
Other requirements
* It is an essential requirement of the role that the post holder has a valid driving licence and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role. However, the Trust would consider making reasonable adjustments to the role, if necessary, to enable a disabled person to undertake the role.
