As a Hospital Discharge Care Support Worker you’ll make a real difference every day to those that have had a hospital stay and are medically fit for discharge to get back into their own homes. You will be travelling to different peoples homes so a driving licence and access to your own car is essential.
Responsibilities
Our Discharge to Assess (D2A) team promotes independence by meeting the individual’s needs. It’s important to us that we support our customers in the community with a collaborative and person-centred approach after their stay in hospital and now back in their own homes.
Our customers will need support with daily living task such as personal care, medication administration, meal preparation, transfers. Encourage physical, mental & social activities, monitor & report on progress and wellbeing in their own homes.

Requirements
Maybe you are an experience Care Assistant and want contracted hours? We are looking for a caring and reliable Hospital Discharge Team members to support individuals in their recovery process after a stay in hospital. You'll play a key role in helping people rebuild confidence and independence in their own homes, working alongside with occupational therapists, district nurses, physio therapist, key workers and other healthcare professionals.
If you care deeply, Bluebird Care makes sure you have the time you need to do what you do best, the training so you can keep doing it better and the rewards that delivering such high-quality care deserves. Must hold a full UK driving licence and have an access to your own vehicle.
