This will involve strengthened multi-disciplinary working with various partners. The aim is to promote best practice, manage demand, and respond to legislative change, with a focus on reablement. We want the people we serve to live good, ordinary lives in the place they call home, with the people and things they love, in communities where they look out for one another. If people need support, we start by understanding what a good life looks like for them, and how we can work together to achieve it. We are looking for dynamic staff with relevant knowledge, skills, and expertise in Mental Health, Learning Disability, Autism, and Forensic Social Work, to facilitate hospital discharge.
