We are delighted to recruit four full‑time Community Care Officers (CCOs) across our South (two) 12‑month fixed‑term and East (two) 12‑month fixed‑term Mental Health teams. The 12‑month fixed‑term roles have the potential to become permanent at the end of the contract period.
Responsibilities
- Managing a diverse caseload of non‑complex and complex mental‑health cases
- Completing strengths‑based Adult Needs Assessments and Reviews in line with the Care Act
- Building effective therapeutic relationships with individuals, families, and carers
- Supporting people to define and work towards meaningful, recovery‑focused outcomes
- Promoting independence, resilience and community inclusion
- Identifying and addressing barriers that impact wellbeing and progression
- Working in partnership with health, housing and voluntary sector colleagues
- Responding flexibly to service demands, including unplanned visits
- Hybrid working: a combination of home and office‑based work, with office attendance as required
- Travel is essential; mileage is reimbursed for journeys from the office to appointments
