In this vital role, youll work directly with customers who are transitioning from hospital or supported accommodation into permanent housing, helping to safeguard their wellbeing, prevent homelessness, and promote independent living.

Key Responsibilities

1. Deliver tailored, person-centred support to individuals with mental health needs, enabling them to successfully transition into and maintain secure, long-term tenancies.
2. Take an active role in multi-agency forums and case conferences to ensure coordinated support and improved outcomes for customers.
3. Identify risks of tenancy breakdown or homelessness early and implement effective, proactive solutions to prevent them.
