Key Responsibilities
1. Deliver tailored, person-centred support to individuals with mental health needs, enabling them to successfully transition into and maintain secure, long-term tenancies.
2. Take an active role in multi-agency forums and case conferences to ensure coordinated support and improved outcomes for customers.
3. Identify risks of tenancy breakdown or homelessness early and implement effective, proactive solutions to prevent them.
