Are you an RMN or OT with community and dementia experience? The Intensive Community Support Dementia team is looking for a Clinical Team Leader to join!
We are a specialist service to which Community Mental Health Teams can refer when patients' behaviours become more challenging. We offer high-quality interventions to help patients in their own environments, including care/residential homes or private residences.
As a Clinical Team Leader, you will be dynamic and highly motivated, with exceptional assessment, risk management, and care formulation skills. Strong communication and clinical decision-making skills are essential. You will undertake shift management to ensure the smooth running of the team, utilizing resources effectively. Since this role covers North and West Dorset and is based in the community, a driver is essential; pool cars are available.
ICSD operates 7 days a week from 07:30 to 20:00 on a rota basis (07:30 - 15:30 / 12:00 – 20:00), including alternate weekends at enhanced pay. This is a full-time role based at Forston Clinic, Dorchester, with opportunities for remote working.
For more information or informal visits, please contact dhc.icsd.westrefferals@nhs.net.
You will collaborate with CMHTs, social care, and other agencies to benefit patients and support carers while patients are under our care. An understanding of MHA, MCA, and safeguarding legislation is essential.
You will share responsibilities with other staff in the Shift Coordinator role (Duty), including triaging referrals, allocating visits, handling phone calls, undertaking assessments, nurse reviews, and managing a small caseload of patients.
You will contribute to and be responsible for implementing service changes guided by the National Dementia Strategy and embracing the Recovery Agenda as it develops.
ICSD is a ‘virtual ward,’ requiring senior nursing staff to process information accurately and swiftly.
Completion of a Prevention and Management of Violence and Aggression course is mandatory; employment is contingent upon successful completion of this course.
