Salary Grade 8, £30,841 to £33,699 per annum
Location: County Hall, Newport, Isle of Wight
These are exciting times on the Isle of Wight within Adult Social Care, the whole service has supported a redesign that enhances strength-based practice, working with the community and our third sector partners. We currently have a Social Work Assistant vacancy in our Wellbeing & Access Hub, supporting people at first point of contact.
This role is an exciting opportunity working alongside Outreach, Carers IW and Occupational Therapy colleagues who form part of this team to help reduce the need for long term care by for example arranging support through our reablement services to help the person regain their independence. It involves offering information, advice and signposting to services in the third sector. You will be working as part of the first point of contact for Adult Social Care, taking referrals by telephone, undertaking visits and Care Act Assessments.
You will be responsible for timely assessments that are strength based, supporting planning that promotes and maintains independence, choice and autonomy of people.
Responsibilities
* Take referrals by telephone, undertake visits and Care Act Assessments as part of the first point of contact for Adult Social Care.
* Conduct timely, strength-based assessments and support planning to promote independence, choice and autonomy for people.
* Implement and promote departmental safeguarding procedures and adhere to recording policy.
* Attend multi-agency discharge planning meetings to ensure timely discharges.
* Support carers and family members, provide information and signposting where required, and offer Carers Assessments in line with the Care Act 2014.
* Engage with Outreach, Carers IW and Occupational Therapy colleagues as part of a multidisciplinary team.
About you / Qualifications
* 37 hours per week.
* Qualified with an up-to-date professional registration.
* Demonstrate skills and experience of promoting a person-centred, strengths-based approach and working as part of a team that places strengths of individuals, families and community at the heart of care and wellbeing.
* Knowledge of The Care Act 2014, Mental Capacity Act 2005, Data Protection Act 1998 and ability to work to practice guidance/procedure.
* Ability to participate in Out of Hours (OOH) standby duty rota as required by management on an occasional basis.
