There is an opportunity for an INT and Digital Care Coordinator to join us at Apollo Primary Care Network (PCN) to develop the way we support our patients, improve staff workflows, and enhance our general practice (GP) services through the adoption and optimisation of current and new technologies. Apollo PCN is at the early stages of the Integrated Neighbourhood Journey and seeks a candidate suitable for a transformational role, working in a diverse team and eager to develop skills to collaborate with system partners.
Main duties of the job
The post holder will support the use of GP clinical systems and digital technologies within primary care to improve local population health and enhance the digital maturity of Apollo PCN and its member practices.

The PCN has multiple projects, and the post holder will support the Clinical Director and team in delivery.
Job responsibilities
* Monitor and oversee the PCN's EMIS system, focusing on workflow, tasks, templates, rotas, and clinical sessions, adapting them to INT needs.
* Use EMIS for searches and audits; develop templates and protocols to support the PCN and practices, contributing to the development of the Neighbourhood Team.
* Run system reports and extract data for activity reporting, using EMIS and other sources like the PHM dashboard, to support INT delivery plans.
* Promote and develop the use of clinical systems and digital products, supporting digital inclusion within the PCN and practices.
* Share best practices across practices, identify IT and information needs, and implement effective systems and training, supporting new IT options appraisal for the Clinical Director.
* Understand clinical system processes and configurations to optimize user experience and system support.
* Develop reports, dashboards, and analysis tools.
* Support digital tools such as PCIT, Accurx, and the NHS App.
* Assist with social media, website, and waiting room screen content management, and develop innovations to enhance communication and delivery.
* Identify anomalies, suggest new ways of working, and flag potential blockers.
* Support digital transformation projects led by the Transformation Lead.
* Contribute to strategic planning and workload management as a member of the PCN Care Coordinator team.
* Act as an RA agent for the PCN if required.

Experience
