We are looking for a Band 5 Community Staff Nurse to join our multidisciplinary Care Community Team, delivering high‑quality nursing care to community patients in rural West Cheshire.
Main Duties
* Provide nursing care and holistic assessment to patients in their homes.
* Coordinate patient care to avoid unnecessary hospital admission.
* Work with professionals to deliver seamless and person‑centred care.
* Travel within the Tattenhall area, using your own vehicle—the Trust reimburses mileage.

Knowledge & Expertise
* Multi‑agency working experience
* Understanding of the Public Health agenda
* Evidence of research‑based and reflective practice
* Knowledge of local and national priorities
* Effective time management skills
* Good interpersonal skills
* Community nursing experience
* Experience with long‑term conditions
Experience
* Highest standards of nursing practice
* Working knowledge of community nursing and multidisciplinary collaboration
* Direct experience in community nursing
Qualifications
* Registered General Nurse, Part 1 of the NMC register
* Evidence of recent training
* Evidence of working towards a degree in Professional Practice or equivalent
Pay
£32,073 to £39,043 a year (pro rata).
