The role:
To provide community-based tier 3/4 NHS specialist mental health assessment and intervention based on the principles of equality, dignity and respect and which takes into account spiritual and cultural diversity.
Within the out-patient setting, care is delivered by multi-disciplinary teams in collaboration with partner agencies. It is based on the best available evidence for practice and is planned and delivered in a manner which promotes personal choice. The clinical team works collaboratively with service users, carers, families and their representatives to ensure that care needs are identified and met.
All children/young people being cared for have a Case Coordinator who is tasked with working closely with their identified patients/carers to develop a participative, therapeutic relationship and deliver high quality specialist care in an environment where patients feel valued and where confidentiality is respected.

Key responsibilities:
The post-holder will be responsible for:
1. Acting as Named Nurse/Practitioner for patients with complex mental health needs, including care coordination.
2. Assessing, planning, delivering, and evaluating holistic, specialist mental health care.
3. Providing safe, least restrictive nursing care and promoting mental and physical wellbeing.
4. Identifying and responding to vulnerability, safeguarding, and risk issues.
5. Developing therapeutic, person-centred, and culturally sensitive relationships with patients and carers.
6. Delivering evidence-based psychological interventions within competency.
7. Administering and monitoring medication and treatments in line with policy.
8. Ensuring compliance with the NMC Code, relevant legislation, and clinical governance standards.
9. Participating in and providing supervision, appraisal, and staff development.
10. Supporting learners and monitoring practice of nursing and support staff.
11. Liaising with statutory and non-statutory agencies across health and social care.
12. Maintaining accurate, contemporaneous records.
13. Providing clinical leadership and management cover in the absence of the Nurse Team Leader.
Knowledge, training, qualifications and/or experience required to do the job:
14. Registered Mental Health Nurse or RNLD - level 1 NMC registration
15. Educated to minimum degree level/ or equivalent
16. Completed a recognised mentorship preparation programme/ registered mentor
17. Specialist knowledge and application of risk assessment/ factors impacting on mental health/ mental illness/ wellness and recovery
18. Evidence of active and ongoing CPD
19. Evidence of Clinical Supervision/ reflective practice as a supervisor
20. Specialist knowledge of rights, values based practice and legislation frameworks
21. Evidence of using digital technology/ information systems/ email and basic applications
22. Experience of working in the community environment
23. Can evidence in career history, the application of a range of specialist skills and behaviours e.g.
24. Organisation and applied leadership skills
25. Self-directing and Self-motivating
26. Enhanced (written/verbal) communication skills including negotiation, influencing, persuading, facilitating and conflict resolution skills
27. Decision making skills
28. Ability to work flexibly and to prioritise workload, delegate appropriately and adapt to meet person centred care without direct supervision
29. Working knowledge and demonstrable evidence of the NMC Code
30. Ability to undertake a number of clinical contacts in a home/ near to home environment on a daily basis to meet which may involve driving
