This is an exciting opportunity for an experienced primarycare navigator / administrator who is enthusiastic,hard-working and motivated with a keen eye for detail.

You will be joining our dedicated, supportive team delivering high quality healthcare services to our patients.

The role is based within our patient services team working in reception.

You will also be processing documents as part of the practice workflow systems.

You will be working closely with all clinical and administrative primarycare professionals, whilst supporting patients with their personalised care and health monitoring requirements.

The practice operates an integrated digital triage system making sure patients are seen by the right clinician, in the right place at the right time.


Main duties of the job

Working as part of our reception team, carry out first line contact with the patients and ensure the smooth running of the reception/administration area between 7.00am 12.00pm.

Liaise with all relevant parties, including face to face, phone contact and emails with patients, clinicians and other health care professionals regarding queries.

To ensure all administration duties and patient requests are processed in accordance with practice policies, confidentiality and health and safety protocols.

Process patients requests and queries

Booking appointments from triaged requests together with face to face and telephone patient contact

Electronic Document workflow

Process tasks and requests from clinicians, admin team and Practice Manager


About us

Centrally located medical centre with a patient list size of 8000.

3 Female GPs, professional team of highly skilled nurses and HCAs, together with our PCN Allied Health professionals

Supportive, dedicated team of administrators and care navigators and coordinators


Job responsibilities

JOB TITLE: Patient Care Navigator/Administrator

REPORTS TO: RECEPTION MANAGER

HOURS: 25 HOURS PER WEEK MONDAY FRIDAY 7.00am 12.00noon

Job Summary:

Working as part of our reception team, carry out first line contact with the patients and ensure the smooth running of the reception/administration area between 7.00am 12.00noon.

Liaise with all relevant parties, including face to face, phone contact and emails with patients, clinicians and other health care professionals regarding queries.

To ensure all administration duties and patient requests are processed in accordance with practice policies, confidentiality and health and safety protocols.

Job Responsibilities:

* Booking urgent and routine appointments from face to face and telephone patient contact
* Process patients requests and queries
* Monitoring and maintaining the computer information for today's appointments
* Ensuring the smooth flow of patients to and from consultation rooms
* Follow up actions requested by the GPs, Practice Nurses, Practice Secretary, Administrator and Practice Manager
* Filing and general administrative duties assigned to you
* To ensure all practice protocols and procedures are followed and maintained
* To attend all training events, this may be outside your normal working hours
* To present yourself in a professional and polite manner at all times.
* Process post
* Support the Doctors, Practice Secretary, Practice Administrator and Practice Manager with any aspect of practice administration and clerical duties as requested.
* Any other duties as reasonably requested.

Person Specification


Experience

* Experience in primary care as reception / administrator
* Excellent Microsoft Office skills especially Excel, Word, Outlook
* Practical experience of working with others
* Experience of using own initiative/multi-tasking


Qualifications

* Good standard of education
* GCSE grade A to C in English and Maths


Skills, Knowledge and Personal

* Be enthusiastic, motivated, resourceful and flexible
* Excellent team player but be self motivated
* Able to demonstrate effective communication skills on an individual, organisational and external basis
* Ability to work with clinicians and administrators
* Possess a professional appearance and manner
* Have drive and enthusiasm
* Self motivated
* Ability to work under pressure
* Car driver/owner to attend meetings or training
* Excellent organisational and time management skills
* Understanding of total triage, NHS App, NHS online
