Job summary
Applications are invited for a part-time Advanced Nurse Practitioner at Gordon Street Surgery. We are a friendly practice with approximately 10,800 patients. The successful applicant will have good clinical, IT and interpersonal skills and be willing to upskill according to need. Previous GP Practice experience is essential, additional training can be offered where needed.
Main duties of the job
Thepost holder will be an experienced nurse working according to the NMC code ofprofessional conduct and relevant guidelines who will provide care for thepresenting patient from initial history taking, clinical assessment, diagnosis,treatment and evaluation of care, demonstrating safe, clinical decision-makingand care for patients within the general practice. They will workcollaboratively with the general practice team and outside clinicians to meetthe needs of patients, supporting the delivery of policy and procedures, andproviding nurse leadership as required. In order to work at this level, NMC requirementsfor advanced practice must be met.
About us
We are a well-established practice in Burton-On-Trent, delivering a wide range of services to our 10,800 patients with the support of our 4 GP Partners, ANP, Clinical Pharmacists, Nurses and Phlebotomist. We are also assisted by Community Nurses and a Community Midwife.
Reception is lead by our Reception Manager and Assistant Reception Manager who supports a team of receptionists. We also have an Operations Manager supporting a team of Medical Secretaries.
We operate from purpose built premises with staff parking facilities.

ClinicalPractice:
1. Triage urgentcare requests, consulting with patients or directing them appropriately.
2. Clinically assessthe patient and diagnose and treat as appropriate, both in the surgery and inthe patients home environment.
3. Provide accurateinformation regarding patient requirements when the patient needs to bedirected to another clinician.
4. Assess, diagnose, plan, implement and evaluate treatment/interventionsand care for patients presenting with an undifferentiated diagnosis.
5. Clinically examine and assess patient needs from a physiological andpsychological perspective, and plan clinical care accordingly.
6. Assess, diagnosis, plan, implement and evaluate interventions/treatmentsfor patients with complex needs.
7. Proactively identify, diagnose and manage treatment plans for patientsat risk of developing a long-term condition (as appropriate).
8. Diagnose and manage both acute and chronic conditions, integrating bothdrug- and non-drug-based treatment methods into a management plan.
9. Prescribe and review medication for therapeutic effectiveness,appropriate to patient needs and in accordance with evidence-based practice andnational and practice protocols, and within scope of practice.
10. Work with patients in order to support compliance with and adherence toprescribed treatments.
11. Provide information and advice on prescribed or over-the-countermedication, on medication regimens, side-effects and interactions.
12. Prioritise health problems and intervene appropriately to assist thepatient in complex, urgent or emergency situations, including initiation ofeffective emergency care.
13. Support patients to adopt health promotion strategies that promotehealthy lifestyles, and apply principles of self-care.
14. Support and manage health needs of women presenting for family planningor sexual health consultation.
15. Assess, identify and refer patients presenting with mental health needsin accordance with the NSF for Mental Health.
16. Participate in vaccination and immunisation programmes, including travelvaccinations, for both adults and children, if required.
Delivering aQuality Service:

Produce accurate, contemporaneous and complete records of patientconsultation, including telephone consultations, consistent with legislation,policies and procedures.

Deliver care according to NSF, NICE guidelines and evidence-based care.
Assess effectiveness of care delivery through self and peer review,benchmarking and formal evaluation.

Utilise the audit cycle as a means of evaluating the quality of the workof self and the team, implementing improvements where required.
In partnership with other clinical teams, collaborate on improving thequality of health care responding to local and national policies andinitiatives as appropriate.
Evaluate patients response to health care provision and theeffectiveness of care.
Support and participate in shared learning across the practice and widerorganisation.
Use a structured framework (eg root-cause analysis) to manage, reviewand identify learning from patient complaints, clinical incidents and near-missevents.
Assess the impact of policy implementation on care delivery.
Understand and apply safeguarding guidelines that support theidentification of vulnerable and abused children and adults, and be aware ofstatutory child/adult health procedures and local guidance.
Ensure the whole team have skills and knowledge regarding domesticviolence, vulnerable adults, substance abuse and addictive behaviour. Provideguidance and support to ensure appropriate referral if required.
