We work for Health and Justice services as part of Central and North West London NHS Foundation Trust (CNWL) and take great pride in the work that we do in a very challenging environment. If you think you would be able to work in a high-pressured setting, delivering all aspects of clinical care, then this could be the role for you.

We are looking for Registered Adult Nurses with senior experience at Band 5 to join us as part of the Primary Care team within the prison of HMP High Down. We are able to offer supervision, training and preceptorship. You do not need to have prison experience as we will provide you with a full induction and training.

Please note that any candidate wishing to apply for a role within our team will have to complete prison vetting clearance. This requires that you have been a resident in the UK for a minimum of 3 years.

Main duties of the job

Primary care is responsible for screening all new arrivals, health promotion, long term condition management, minor ailments and responding to incidents wherever they may occur in the prison. Working alongside the Mental Health and Substance Misuse team affords us the benefit of being able to work in a truly holistic manner, responding to the needs of both physical and mental well-being in partnership. Our other partners include; GP, dentistry, optician, podiatry, physio and radiology. We also work closely with HMPPS staff, education and other professionals.

Our population is one which is either excluded from mainstream healthcare services or have very limited interaction with health services in the community. Your interactions could influence the way a man thinks about his future, thinks about his health needs, and feels about himself and others. You could make a real difference.

Detailed job description and main responsibilities

9. Plan, deliver and evaluate clinically appropriate interventions using evidenced based guidance and practice.

10. Set up processes/protocols for effective assessment, planning, implementation and evaluation of care for those patients with physical, mental health and substance misuse problems.

11. Safeguard vulnerable patients in line with CNWL Trust safeguarding policies.

12. Apply local risk assessment and management protocols.

13. Make appropriate referrals to other services for long term or complex care management.

14. Co-ordinate multi-disciplinary clinical practice to provide focused and integrated care treatment programs/packages.

15. Use highly skilled interventions to de-escalate aggressive patient behaviours and be aware of the appropriate processes for dealing with them.

16. Work collaboratively with staff of other disciplines e.g. prison, education staff to provide integrated and coordinated care.

17. Work within own sphere of competency and ensure that training needs are communicated to the team manager.

18. Participate in local and trust audits and trust audits. Participate in clinical governance and utilise research skills for audit, service development and research within the area served by the multi-disciplinary team.

19. Liaise with other professional services e.g. hospital staff, GPs and caseworkers, sharing information where appropriate.

20. Work in a highly emotive environment with patients who may have a sexual offending history or mental health / substance misuse problems on a regular basis. Able to communicate sensitive and complex information whilst maintaining a high level of professionalism at all times.

21. Attend and contribute to prison meetings as required, and present the view of Healthcare Team on relevant clinical issues. Ensure any actions taken from these meetings are communicated to the appropriate individual responsible for completing the action.
22. Ensure that pathways of care are created for the patients, including collecting collateral information and making appropriate referrals into the community when they are approaching discharge.

23. Work closely with colleagues to ensure information about risk is recorded and communicated sensitively and comprehensively.

24. Maintain the highest standards of clinical record keeping, including electronic data entry and recording.

25. Ensure that within all aspects of professional activity e.g. communication with patients, relatives, colleagues and the assessment, development and delivery of interventions, issues of diversity such as race, culture, identity, gender and sexuality are sensitively considered and incorporated to reflect individual differences appropriately.

Research and Audits

26. Utilise theory, evidence-based literature and research to support evidence-based practice in individual work.

27. Assist in audits as per the annual planner or when requested to do so in order to develop and improve service provision.

Teaching, Training and Supervision

28. Participate in regular clinical and managerial supervision in accordance with professional practice and CNWL guidelines.

29. Attend appropriate business and clinical meetings, supervision and reflective practice groups

30. Develop skills and competencies that assist in the delivery of current duties

31. Contribute to and facilitate the training and support of all staff.

32. Manage and provide supervision and training to nursing staff.

33. Ensure all appraisals are up to date and each member of the team has a personal development plan.

34. Develop nursing skills and systems which allow for effective triage, minor aliment management and appropriate referral to other healthcare services.

35. Participate in the induction and training of new staff, peers, junior staff and students

Management, Recruitment and Service Development

36. Assist in the design and implementation of service development projects within the service as required.

37. Attend meetings in which service developments are planned and discussed.

IT responsibilities

38. Input and maintain appropriate electronic clinical records and statistical data, providing this to the Service Lead/business and performance team when necessary.

Policy and Procedures

39. Duties and responsibilities of this post will be undertaken in accordance with the policies and procedures of CNWL / Health & Justice Service Directorate and with the NMC Code of Conduct and Guidance on Professional Practice. It is the post-holder’s responsibility to ensure that they keep up-to-date with these policies and other policy documents.

40. Works in accordance with Health & Justice Service Directorate procedures, in particular security, health and safety.

41. Applies equal opportunity principles at all times, participate in training activities and Individual Performance and Development Reviews.

General

42. Contribute to the development and maintenance of the highest professional standards of practice, through active participation in internal and external CPD training and development programmes, in consultation with the postholder’s professional and operational manager(s).

43. Maintain the highest standards of clinical record keeping and report writing in accordance with professional codes of practice and Trust policies and procedures.

44. Perform other duties of a similar kind appropriate to the grade, which may be required from time to time by their manager.
