Cancer Support Worker - Acute Oncology and Cancer of Unknown Primary
Detailed job description and main responsibilities
The CSW will undertake specifically identified tasks for which they have been trained, can demonstrate skill and understanding and are assessed as competent.
These include, but are not limited to:
1. Communicating with cancer patients, their family, friends and carers, both face-to-face and on the phone
2. Liaising effectively with the MDT and other healthcare professionals to provide high-quality coordinated patient-centred care
3. Providing evidence-based patient information according to patient need
4. Assessing patients' emotional and practical needs and responding appropriately
5. Undertaking Holistic Needs Assessments and making appropriate referrals
6. Providing support for administration and management of Health and Wellbeing Information and Support
7. Accessing appropriate IT systems
8. Signposting patients to resources and support for self-management
9. Assisting in coordination of patient treatment pathways
10. Assisting in coordination of care for patients on a follow-up pathway
11. Maintaining database to monitor workload
12. Recording of nursing activity
13. Auditing and evaluation of follow-up pathways, Health and Wellbeing Information and Support and patient outcomes
Person specification
Qualifications
Essential criteria
* Educated to GCSE A level or equivalent AND NVQ Level 4 or equivalent in Healthcare
* OR equivalent experience across a range of health or social care settings at AfC Band 3 or above
* Care Certificate
Knowledge
Essential criteria
* Knowledge and previous experience of using hospital computerised systems
* Insight into cancer and related issues
* Understanding of health and social care environment
Experience

* Experience working within clinical, health or social care setting
* Administrative tasks
* Dealing with routine and complex problems
* Experience working within guidelines, protocols and policies
Skills
Essential criteria
* Proficient communication skills, verbal and non-verbal
