About the Role

Social Care Reablement is a specialist Homecare team that work across the city to provide support to citizens in their own homes. This is mostly acute, short-term care for citizens who have recently been discharged from Hospital or another service. The focus of the work done by the teams is to help citizens reintegrate back into their previous lives, whether that is socially, physically or emotionally.

The teams encourage independence and choice for citizens, and control over the way their care is provided. This involves not only providing personalised care as directed in the citizen’s care plan, but also encouraging and enhancing the wellbeing of citizens through promotion of independence, early intervention, community support and friendships.

About You

The ideal candidate will have:

* Experience of undertaking personal care tasks (e.g. assisting with washing, bathing, dressing, feeding and using the toilet etc.) to a high standard in a respectful and caring manner;
* Ability to assess social care and health needs and develop, implement, maintain and update Person Centred Care Plans;
* Knowledge and understanding of the rights, needs and feelings of vulnerable adults from diverse social and cultural backgrounds;
* Ability to administer medication in line with departmental policies and procedures.

Hours will be worked according to a rota, and this will include weekend work. A valid driving licence and access to your own vehicle is essential for this role.

In your personal statement, make sure you consider the following points:

* Describe how you might have cared for or provided support for someone in the past (this can include paid, voluntary or support for relatives);
* Describe what kind of person you are and what qualities you have, that make you good at care work.
