Overview
We are looking for a dedicated and dynamic Locality Social Worker to join our team. The ideal candidate will have a passion for supporting individuals and families within their community, providing comprehensive case management, and working collaboratively with other professionals. This role requires excellent communication skills, a strong understanding of social work principles, and the ability to manage a diverse caseload effectively. Up to £35 per hour dependant on expereince.
Responsibilities
* Assess the needs of individuals and families within the community.
* Develop and implement personalized care plans.
* Monitor and review progress, making adjustments to care plans as necessary.
* Maintain accurate and up-to-date case records.
* Provide emotional and practical support to service users.
* Conduct home visits, assessments, and reviews.
* Facilitate access to resources and services, such as housing, healthcare, and financial assistance.
* Advocate for the rights and needs of service users.
* Work collaboratively with other social workers, healthcare professionals, educators, and community organizations.
* Participate in multidisciplinary meetings to ensure coordinated care.
* Develop and maintain strong relationships with local agencies and service providers.
