Patient and Service Coordinator
Role at TQI Solutions within the PML Collaborative Care Team (CCT).
Job Summary
PML’s CCT supports seventeen General Practices in West Northamptonshire, providing proactive assessment and support to people with long‑term and complex conditions and their carers. The team operates Monday to Friday, 08:30‑16:30 (excluding bank holidays). This role coordinates the daily delivery of the CCT service, acting as the central point of contact for the team, patients, and carers.
About The Collaborative Care Team
The CCT currently comprises two Registered Nurses (RNs), an Occupational Therapist (OT), two Assistant Practitioners (AP), and a Social Prescribing Link Worker. Patients praise the team for knowledgeability, equipment provision and support with accessing benefits such as Attendance Allowance and Lifeline.
Main Duties
* Provide daily coordination of the CCT service delivery under the direction of the Clinical Lead, ensuring appropriate support is available to the team, patients and their carers.
* Liaise with multidisciplinary professionals and colleagues to facilitate a coordinated approach and keep everyone updated on issues or concerns. This includes active participation in MDT meetings within the CCT, PCNs and practices.
* Provide administrative coordination for the CCT service delivery supporting PML Governance, operational and performance compliance.
* Develop expertise in clinical administrative systems (SystemOne and EMIS), including problem solving, staff support, access, reporting and contingency.
* Maintain accurate, up‑to‑date records using the CCT clinical system, GP and other relevant records, adhering to information governance and data protection legislation.
* Support the Clinical Lead to ensure that PML/CCT remains safe and effective, using audits and learning from significant events and complaints, including collating evidence for CQC compliance.
Person Specification
Experience
* Desirable: Working within a health and/or social care environment and with clinical systems.
* Desirable: Working as part of an integrated multi‑skilled team to optimise patient care and experience.
Qualifications
* Essential: GCSE or equivalent in English & Maths.
* Desirable: NVQ Level 3 in health and social care.
Skills and Knowledge
* Excellent written and verbal communication skills.
* Proficiency in relevant IT systems and software.
* Strong organisational and time‑management skills.
* Good interpersonal skills; ability to build effective relationships with others.
* Ability to work in a changing environment and demonstrate coping under pressure.
