Job summary
An established primary care organisation is seeking an experienced Practice Nurse with a background in chronic disease management to work across multiple GP practices within a Yeovil-based Primary Care Network. The role focuses on delivering high-quality, patient-centred care and supporting long-term condition management within the community.
Main duties
Manage and deliver long-term condition clinics
Diagnose, monitor, and review patients with chronic diseases
Develop, implement, and update personalised care plans
Complete QoF and enhanced service annual reviews using clinical templates
Provide same-day acute patient reviews when required
Process pathology results, workflows, and support clinical audits
Refer patients to multidisciplinary services including social prescribing and wellbeing teams
Supervise, support, and train other members of the clinical teamPerson specification
Essential
Registered Nurse with current NMC registration
Qualifications and experience in chronic disease management
Minimum of 2 years' recent experience managing patients with long-term conditions
Strong communication, clinical decision-making, and care coordination skills
Knowledge of local and national health policyDesirable
Mentor or teaching qualification
Clinical supervision training and experience
Knowledge of EMIS, SystmOne, and Ardens
Non-medical prescribing and advanced clinical skillsAbout the organisation
