Responsibilities
* Deliver evidence-based dietetic services tailored to residents’ nutritional needs and medical conditions.
* Manage a varied caseload, including cases related to frailty, diabetes, oncology, and more.
* Perform comprehensive dietetic assessments and implement personalised nutritional care plans.
* Communicate effectively with residents, families, GPs, and care teams, promoting a collaborative care approach.
* Respond promptly to urgent referrals and support care continuity outside of regular hours when necessary.
* Provide clinical input into the prescription of oral and enteral nutrition, in line with best practices.
* Coordinate care plans for individuals transitioning between hospital and home.
* Partner with the Chef Team to enhance menu offerings, aligning with nutritional standards and residents’ needs.
