An exciting opportunity has arisen for experienced HCSWs to join our Intermediate Care Team based in St Andrews.
The Intermediate Care Team provides services to the adult population within the St Andrews locality, delivered either in service users' own homes or, when necessary, as out-patient care.
The primary responsibility of this role is to collaborate with Occupational Therapy and Physiotherapy staff to deliver therapeutic treatment programmes, aiming to prevent hospital admissions and support early hospital discharges. You will manage your own caseload, assessing, progressing, and discharging patients autonomously within set parameters.
Applicants should have an HNC/HND in Health and Social Care or equivalent, or SCQF level 8 qualification (e.g., Postural Stability Instructor). Experience as a therapy support worker and working with older adults are essential.
Key skills include excellent organisational and communication skills, with the ability to work independently and as part of a multidisciplinary team.
This community post may require work at other hospital sites, and public holiday work is involved. Travel throughout the area is essential.
