Job Summary
We are looking for a GP to join our well established, vibrant and forward‑thinking practice in the heart of Mid Sussex. Candidates will be focused on providing great patient care whilst being dynamic and adaptable to meet the changes we will see in general practice over the coming years.
Main Duties
We currently have the opportunity for a GP to join us for 6-8 sessions per week initially salaried with a view to partnership.
About Us
We are a training practice led by a strong partnership and management team with salaried GPs, ANPs and a range of other clinical staff to support us in our roles. We pride ourselves in providing a supportive working environment for all and meet regularly with the MDT to ensure a holistic approach to our care and opportunities for ongoing learning for all staff.
Cuckfield and The Vale Practice looks after 13,000 patients and is a dual site surgery based in Cuckfield and Haywards Heath. We are an active member of the flourishing Haywards Heath Villages PCN. All staff work over both sites and our visiting rates are low. We use SystmOne and Ardens and hope to grow our list size over the coming years. We are high QOF achievers.

Job Responsibilities
* Delivery of highly effective medical care to the entitled population
* Provision of services commensurate with the primary care contract
* Clinically examine and assess patient needs from physiological and psychological perspectives and plan clinical care accordingly, referring to other services/agencies in a timely manner and utilising practice and local guidelines
* Generic prescribing adhering to local and national guidance
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
* Effective management of long‑term conditions
* Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self‑care
* Processing of administration in a timely manner, including referrals, repeat prescription requests and other associated administrative tasks
* Review medications following appropriate policies, NICE/local clinical guidelines and local care pathways
* Maintain accurate clinical records in conjunction with good practice, policy and guidance
* Work collaboratively, accepting an equal share of the practice workload
* Adhere to best practice recommended through clinical guidelines and the audit process
* Support the delivery of anticipatory care plans
* Contribute to the successful implementation of continuous improvement and quality initiatives within the practice
* Accept delegated responsibility for a specific area (or areas) of the QOF
* Attend and contribute effectively to practice meetings as required
* Contribute effectively to the development and maintenance of the practice including clinical governance and training
* Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on safeguarding adults and children
* Ensure compliance with the appraisal process
* Prepare and complete the revalidation process to remain fit to practice
* Commit to self‑learning and instil an ethos of continuing professional development across the practice team

Person Specification – Qualifications
* Qualified GP
* MRCGP
* Vocational Training Certificate or equivalent JCPTGP
* Certificate of Completion of Training (CCT)
Person Specification – Experience
* Ability to work autonomously and as part of a multi‑disciplinary team
* Experience of working in a primary care environment
* Experience of providing home visits
* General understanding of the primary care contract
* Proven ability to evaluate the safety and effectiveness of own clinical practice
* Appreciation of the NHS landscape and relationships between organisations, PCNs and commissioners
