This role spans across St Mary’s Hospital, Charing Cross Hospital, and Ealing Community, with occasional cover at Hammersmith Hospital.We are looking for a Registered Nurse with:- Cardiac experience, ideally across both cardiology and cardiothoracic services. - A strong interest in cardiac rehabilitation and secondary prevention.- Excellent communication and organisational skills.- The ability to work independently and collaboratively within a multidisciplinary team.- You should be passionate about patient-centred care, committed to sharing best practice, and keen to contribute to service development and innovation.Main duties of the job- Support the cardiac rehab team day to day to ensure the programme runs effectively and efficiently.- Provide in-patient services when required to identify and recruit patients and their families.- Assess the risk factors and needs of patients with cardiovascular disease and plan an agreed package of care with the patient.- In line with the cardiac rehab protocol undertake full nursing assessment, implementation and evaluation of patient, and families/carers. to provide specific expert nursing care, advice, support and counselling where necessary.- To communicate complex information following assessments and analyse information which is sensitive to patients/carers and health professionals.

- Support the supervised exercise sessions of the cardiac rehab programme and deliver health education sessions as required. - Participate in the co-ordination of patient care across the patient pathway, developing the pathway across the various settings to maximise patient satisfaction and outcomes.Working for our organisationAt Imperial College Healthcare you can achieve extraordinary things with extraordinary people, working with leading clinicians pushing boundaries in patient care. For both overviews please view the Job Description attachment with the job advert.Person specificationEducation/ QualificationsEssential criteria- Post Registration qualification in Cardiology/ related subject- First Level Registration- ENB 998 or equivalent- ILS trainedExperienceEssential criteria- Minimum 2 years recent experience of working with people following a cardiac event.

Experience of working in a cardiac rehabilitation team- Experience in teaching, coaching or mentoring- Delivering Health Education to individuals and/or groups- Experience of auditing and evaluating clinical nursing careSkills/Knowledge/ AbilitiesEssential criteria- Understanding of Cardiac Rehabilitation programmes and secondary prevention- Knowledge of national policies & directives that impact on service delivery (BACPR, NICE)- Understanding of the principles of health education and motivational interviewingRight to workIf you need sponsorship to work in the UK, please visit the Home Office website for information on sponsorship and visa status before you fill in your application form.
