Job summary
AMP Group are looking for enthusiastic, hard working people to join their team as a care coordinator for between 30-35 hours per week.
Care Coordinators work alongside administration and clinicians to ensure the management and review of patients with long term health conditions. They complete a range of administrative tasks including, cancer screening reminders, review invites. They also assist in the taking of bloods and patient observations.
This is a very varied role which requires excellent customer service, excellent IT skills and a willingness to learn.
Main duties of the job
1. Proactively identify and work with people, including the frail/elderly and with long term conditions
2. Provide navigation of care and support across health and care services
3. Focus on supporting and delivery of personalised care to reflect local PCN priorities, health inequalities or at risk group patients.
4. Ability to run searches in the clinical system for meeting target specific goals and objectives
5. Ensure patients are contacted and directed to the right service based on practice call/recall policy and clinical system alerts
6. To assist in the monitoring of the practices performance against QOF & IIF compliance on a monthly, quarterly and annual basis
7. To assist in the monitoring of compliance with local and national enhanced Services, Vaccination & recall programs
8. Dipping urine, taking blood pressure, phlebotomy and other observations and clinical tests
9. Extracting all information from clinical letters that needs coding and adding to notes
10. Arranging appointments, referrals, tests and follow up appointments of patients
11. Managing digital services, potentially including the practice website/social media
About us
Our premises are modern, and purpose built. Our team consists of GPs, Advanced Clinical Practitioners, Practice Nurses, a Nurse Associate, HCAs and Phlebotomist. We use SystmOne, Ardrens and Accurx software. We are a well-established practice for over 13,500 patients over four sites and our list size is growing year on year.
Through our PCN membership we have had opportunity to work collaboratively across our Network area. We have had opportunity to work alongside a number of ARRS roles including Pharmacy Team, Physiotherapist, Primary Care Mental Health team, as well as a Social Prescribers. We have a very supportive and enthusiastic Management team, excellent Patient Services and Admin support staff.
