Job Overview
To work as an autonomous practitioner within an integrated team, supporting the delivery of the EPUT vision of patients being seen by the right person, with the right skills at the right time. To lead a team of mixed skill community nursing staff to manage appropriate care for patients in their own home/community setting. To manage the complex role of Community Nurse over a defined area, with a commitment to the health improvement of individuals, families and the community. The post holder is required to comply with all EPUT Policies and Procedures and the NMC Code. The post holder is required to take responsibility for own professional development, identify needs through annual appraisal and maintain a Personal Development Plan.
Responsibilities
* Lead an integrated community nursing team under the guidance of the Modern Matron and Senior Community Lead Nurse.
* Provide assessment, planning, treatment and delivery of nursing care in the community.
* Develop strategies to promote health and independence while recognising cultural beliefs and values of individuals.
* Prevent unnecessary hospital admission and enable patients to stay at home where possible.
* Supervise, support and teach junior staff.
* Work within the policies of the Trust and the NMC Code.
* Liaise closely with multi‑disciplinary teams as part of collaborative working to support the case management of patients.
* Maintain professional development through annual appraisal and a Personal Development Plan.
Qualifications
Registered General Nurse at first‑level who has at least three years of community nursing experience. Demonstrated leadership and ability to work within a changing environment, flexibility and a willingness to develop with service needs.
