The Community Rehabilitation and Reablement Service provides specialist rehabilitation across East Renfrewshire HSCP. The multi-disciplinary team consists of physiotherapy, occupational therapy, dietetics, speech and language, nursing, Assistant Practitioner and rehabilitation support worker intervention. The Service operates over two geographic teams in Eastwood and Barrhead localities and provides supported discharge from Acute settings and coordinated assessment and treatment within an individual's own home (or other appropriate community or workplace setting) to maximise health gain and independence.
Staff work as part of a flexible and responsive team which aims to provide a single point of access to assessment of need followed by time-limited and appropriate rehabilitation. Close collaboration exists with service users, carers, primary and secondary healthcare, social work services and the voluntary sector.
Essential to this post:
Experience of working with Adults and Older People in a community setting.
Experience as a therapy assistant/ rehabilitation support worker
SVQ Level III certificate and/or minimum of 2 years experience working a within Health and/or Social Care setting.
A current full driving licence.
Effective verbal and written communication skills.
Trained in mobility assessment, progression of mobility, exercise classes, passive movements, individual exercise programmes.
Ability to work independently
