The role: You as the Social Worker will be to assess and where necessary, provide short and long term support to individuals to promote their independence Your role as Social Worker will involve working with a wide range of cases for all adults from the age of 18 As the Social Worker you will effectively manage a varied caseload with appropriate supervision, guidance, and support, reflecting the function of the team and Adult Social Care in accordance with policy, procedures, guidance and legislation As part of your role as a Social Worker you will complete Your Needs Assessment, Risk Assessment, Mental Capacity and Best Interest assessment, completing Reviews, as well as any other relevant assessment in relation to a person’s needs, as well as be able to commission services such as Domiciliary Care packages, Day Centre placement. Carers support including carers assessments. Assistive technology, minor adaptations etc Social Workers will work using a Multi-Disciplinary approach and will be required to attend MDT meetings with families and relevant professions, participate in Duty responsibilities and work together with social workers, Senior practitioners and Team Manager Through every interaction, you the Social Care Assessor will apply the Prevent, Reduce, Delay principles of the care act to promote independence and wellbeing Hospital Discharge Team – In addition to the above, The Hospital Discharge team is a vital service focuses on enabling safe, timely, and person-centred hospital discharges by providing early intervention, short-term support, and robust assessments. The team works collaboratively with Nurses, Therapists and health colleagues to deliver a strengths-based response that promotes independence and reduces reliance on long-term care.
