Overview
We are a very friendly team based in Halesowen who serve 5800 patients. Our team includes 3 GP partners, 2 salaried GPs, a practice nurse and HCA. We are looking for an adaptive, motivated and hardworking individual who can bring their knowledge and expertise to the role of clinical coder, summariser, admin support and actioning GP referrals. The practice values high clinical standards and aims for an excellent patient experience.
Responsibilities
* Summarise and add SNOMED codes for relevant information from the patient’s clinical records into the practice EMIS system.
* Ensure diagnoses and investigations are reflected accurately on a patient’s medical record.
* Organise new patient records and letters in preparation for summarising; review medical records and produce an accurate summary of the patient’s medical history, identifying higher risk areas that require input.
* Read all incoming correspondence to the practice, accurately coding relevant diagnosis/procedure/information, deciding whether to send to a GP, and ensure follow-up actions/tests are brought to the attention of the relevant staff.
* Assist GPs with the flow of correspondence (e.g., hospital letters, reports, other patient communications); identify documents to send to the Clinical Team for review (e.g., medication changes, hospital results).
* Scan patient-related documents onto their medical records using codes as agreed by the clinical team; ensure incoming letters are scanned onto the patient record.
* Ensure that all QOF-related data is captured and that clinical codes are current.
* Follow practice protocols to ensure consistent and high-quality care for patients.
* Action referrals on the Choose and Book system as recommended by the clinical team.
* Provide admin support to the rest of the admin team where required.
Qualifications
Essential
* Completed medical terminology course
* Experience with using EMIS and Docman
* English literacy and numeracy skills to GCSE level C or above
* Experience in actioning referrals on the Choose and Book system
Desirable
* Experience in working in General Practice
