Oath Healthcare is recruiting experienced Support Workers to work with adults who are homeless or at risk of homelessness, and adults with learning disabilities and complex needs. You will be supporting people in supported accommodation, hostels, community services and small residential settings to develop independence, improve wellbeing and move towards their goals.
Key responsibilities

Provide person‑centred support with daily living skills (budgeting, cooking, appointments, tenancy sustainment, personal care where required).

Work with individuals experiencing mental health issues, substance misuse, offending history or trauma, using de‑escalation and professional boundaries.
*
Support adults with learning disabilities and autism with communication, personal care, activities, community access and skills development.
*
Develop and follow support plans and risk assessments; record accurate daily notes, incidents and outcomes.
*
Promote safeguarding, dignity, choice and inclusion at all times.
*
Work collaboratively with housing officers, social workers, health professionals and families.
Requirements
*
Previous experience in supported housing, homelessness services, hostels, learning disability services, supported living or similar is essential.
*
Strong communication and resilience; able to work with people who may present challenging behaviour or be in crisis.
*
Understanding of safeguarding, professional boundaries and confidentiality.
*
Right to work in the UK, satisfactory references and Enhanced DBS (or willingness to obtain one).
*
Flexibility to work shifts including evenings, weekends, sleep‑ins or nights as required.
