We are excited to announce a rare opportunity to join our wonderful team! We have a two permanent vacancies within our Patient Journey Department.
Key Responsibilities
One Peripatetic Patient Journey Facilitator role will be based at The Princess Royal Hospital, Telford and the other will be based at The Royal Shrewsbury Hospital, Shrewsbury; cross‑site working will be required to support the service. Flexibility is essential to provide additional support to areas where demand exceeds usual activity or where there is annual leave or short/long‑term absence.
The successful candidate will act as the main point of contact regarding the discharge of patients on ward areas, linking with social care and therapy services where appropriate. Strong communication and organisational skills are required to ensure tasks are completed and concerns are escalated promptly.
Ideally, the applicant will have an understanding of the SAFER Red2Green approach and the various pathways to ensure patients’ journeys progress in a timely manner.
Daily updates to the database must be performed to ensure patients are discharged on schedule. When discharge criteria are not met, proactive steps should be taken to bring investigation and treatment schedules back within target.
Monitoring individual patient pathways, tracking progress, and ensuring timely intervention are key duties.
Attend daily board rounds and identify patients who require the support of social services, Pathway Zero, or other support services to facilitate discharge.
Ensure the accuracy of information available on the Careflow and Patientflow systems and update or progress records as necessary.
Provide flexible cover in the event of a vacant Patient Journey Facilitator post or in cases of long/short‑term absence. Support fellow facilitators during high discharge volumes and work flexibly across wards as required.
Team Context
The Patient Journey Facilitator’s role sits within the Medicine and Emergency Care Division – Patient Journey Department, alongside the Medicine Flow Coordinators. The team is supported by the Medicine Operational Managers who assist with ward support related to patients’ journey and discharge processes.
