Care Manager (Healthcare or Clinical Background Preferred)
We are looking for a confident, organised and compassionate professional to oversee the safe, effective and person‑centred delivery of care across our services. The role is ideal for someone with a Registered Nurse, Physiotherapist or Occupational Therapist background who can apply their clinical expertise within a community‑based setting.
The Role
As a Care Manager, you’ll play a central role in overseeing the safe, effective and person‑centred delivery of care across our services, ensuring people receive the right support, at the right time, in their own homes.
Your responsibilities will include:
* Managing a caseload of clients, ensuring care plans, risk assessments and reviews are accurate, up to date and outcomes‑focused
* Providing leadership, guidance and support to care staff to maintain high standards of care
* Carrying out assessments, reviews and quality monitoring visits
* Acting as a key point of contact for clients, families, care teams and external professionals
* Ensuring care delivery remains compliant with CQC standards, safeguarding requirements and company policies
* Identifying concerns early and taking appropriate action to maintain safety and quality
* Supporting service development, audits and continuous improvement activities
As a Care Manager, your professional judgement and leadership will make a real difference. About You
You’ll bring:
* Experience in care management or a healthcare/clinical setting, with strong professional judgement
* A person‑centred approach and a clear understanding of dignity, choice and independence
* The ability to build positive relationships with clients, families, care staff and external professionals
* Strong communication, organisation and decision‑making skills
* Confidence identifying and managing risk, safeguarding concerns and quality issues
* A proactive, solutions‑focused approach to supporting teams and improving care delivery
* Have a full UK driving licence
