Role Description
This is a full-time on-site role for a Complex Community Branch Nurse, located at Derby Branch. The Complex community Branch nurse plays a key role in delivering high-quality complex health services by providing clinical supervision, client assessment, care planning, and staff support within allocated caseload. The role ensures that all care provided meets regulatory standards, clinical best practices, and organizational policies to promote patient safety and satisfaction. The Branch Nurse is responsible for managing their own caseload of clients, ensuring that each receives appropriate, person-centered, and timely nursing input while maintaining effective communication with the wider multidisciplinary team.
Key Responsibilities
Clinical Care & Oversight
* Conduct initial and ongoing assessments of patients in their homes.
* Develop, implement, and review individualized care plans in collaboration with the multidisciplinary team.
* Provide clinical guidance and support to caregivers, healthcare assistants, and other staff.
* Complete training and competency sign-offs for care staff to ensure safe and effective care delivery.
* Ensure accurate and timely documentation of all care activities.
Quality & Compliance
* Maintain compliance with local health authority regulations and company policies.
* Participate in audits, risk assessments, and incident reviews.
* Ensure infection control and safety standards are consistently met.
Training & Supervision
* Provide orientation and continuous training for homecare staff on clinical skills, documentation, and patient safety.
* Supervise and evaluate field staff performance and competency.
* Support professional development and promote a culture of clinical excellence.
Coordination & Communication
* Liaise with physicians, hospitals, families, and other healthcare professionals to ensure seamless continuity of care.
* Support branch management in service delivery, scheduling, and patient assignments.
* Participate in case conferences and multidisciplinary meetings.
Administrative Duties
* Assist in maintaining branch records and reports related to clinical operations.
* Support recruitment and competency evaluation of clinical staff.
* Contribute to branch goals and initiatives to enhance service quality and patient satisfaction.
Qualifications
* Nursing credentials, both practical and theoretical knowledge
* Experience in complex care, disease management, and administering medication
* Ability to develop and update care plans
* Strong communication and interpersonal skills, including the ability to work collaboratively with multidisciplinary teams
* Proficiency in maintaining patient confidentiality and professional standards
* Compassionate and empathetic approach to patient care
* Flexibility to adapt to the needs of the clients
* Experience in rehabilitation and reablement services is a plus
* Relevant certifications and licenses for nursing
