POSITION TITLE: Patient Care CoordinatorSTATUS: Non – ExemptREPORTS TO: Clinical Support SupervisorDIRECT REPORTS: N/AACCOUNTABILITY OBJECTIVE: Responsible for providing high-quality administrative support to enhance patient care and satisfaction. Serves as a liaison between clinical staff, patients, providers, and pharmaceutical representatives.KEY RESPONSIBILITIES: • Provide patients with assistance with insurance authorizations on medications for coverage under their insurance plan. • Work with patients and pharmaceutical representatives to obtain information necessary for medication coverage. • Verify patient’s pharmacy insurance eligibility • Effectively utilize the EMR as well as online authorization sites for medication programs, i.e. CoverMyMeds • Provide necessary forms for patients for completion and signature, review all forms for completeness and accuracy. • Telephone triage • Respond to patient phone calls and concerns • Process RX refills that come from the pharmacies or patients • Arrange for consults and outside testing, such as MRI, other imaging, lab work, outside sleep studies using EMR system. • Complete medical records requests • Process and respond to patient requests via our patient portal • Assist MAs and providers as needed. • Other job duties as assigned.QUALIFICATIONS AND SKILLS: • Active LVN/ LPN or Certified Medical Assistant for a minimum of 7 years. • Associates or Bachelor’s Degree (Preferred) • Up to date on CPR training • Excellent verbal and written communication skills to include superior interpersonal and patient contact skills and excellent telephone manners. • Ability to effectively lead a teamADDITIONAL EXPERIENCE, NOT REQUIRED: • At least one (2) year of related experience in a Neurological setting • Ability to communicate effectively and handle multiple priorities • Strong computer and typing skills.If you have an appointment scheduled at our clinic or Infusion Center in Bedford on or after 10/14/2025, we will see you at the new location.
