The purpose of the role is to provide high quality specialist palliative practitioner care, skills, advice and support to patients, carers and those important to the patient.
Main duties of the job
1. Actively participate in the development of the organisation's planned and unplanned community model of care.
2. Offer advice, support and education to the primary health care team as part of an interdisciplinary team.
3. Manage a specialist palliative planned care caseload within a set Primary Care Network (PCN) and provide cross-cover for other surgeries as required.
4. Work from an interdisciplinary approach to care.
5. Engage in activities consistent with the role of a Clinical Nurse Specialist/Clinical Paramedic Specialist.
6. Demonstrate specialist skills in palliative care, delivering high standards of personalized care.
7. Improve patient outcomes and experience by providing physical and emotional support, coordinating care services, and advising on clinical and practical issues.
8. Participate in audits and case reviews to enhance the Hospice @Home service.
9. Act as a community ambassador for Rennie Grove Peace Hospice Care, promoting its values and services.
10. Support safe and effective service delivery across the geographical area as needed.

Rennie Grove Peace Hospice Care was formed in July 2023 through the merger of Rennie Grove Hospice Care and Peace Hospice Care. We provide specialized care for those affected by life-limiting illnesses or bereavement in Herts and Bucks, aiming to expand our services, reach more people, and strengthen resource acquisition.

Person Specification
Essential Qualifications:
* First level Registered Nurse or Registered Paramedic
* Degree or modules in Palliative Care or related field
* Proven palliative care experience
* Non-medical Prescriber or committed to obtaining qualification
* Advanced assessment and communication skills training
* Leadership experience
* IT literacy
* Excellent communication and organizational skills
* Teaching and presentation skills
Desirable Qualifications:
* Experience in clinical teaching
* Experience supporting change and leading volunteers
* Problem-solving skills
