Reablement Assistant Practitioner - 7117 | Permanent contract | £31,537 to £33,699 per annum (Scale G) | 37 hours per week | Priory House, Kings Lynn

Would you like to work in the community as part of a well-trained team of Occupational Therapists and support workers, creating person-centred personal plans to help people set and achieve goals after an illness? This role is for you.

Due to expansion, there are opportunities to visit people in their own homes to assess social care needs across a range of locations. Norfolk First Support delivers services to adults in their own homes for up to 6 weeks, assisting them to regain skills they may have lost following illness or hospital admission, enabling them to regain independence as much as possible while reviewing ongoing support needs.

In this role you will be part of a multidisciplinary assessment team, working with other professionals in Social Care to support people who may have been discharged from hospital or whose circumstances require home support.

You will assess and review daily living needs, developing a personalised plan to help individuals regain skills and confidence to remain independent at home. If ongoing services are required, you will complete a Care Act Assessment. All training will be provided to ensure you are competent to deliver this critical role.

You will need experience of working with people, the ability to communicate clearly and appropriately with members of the public, and the ability to plan and prioritise your work using your initiative. Knowledge of current policy, practice and legislation within Health and Social Care is required.

We operate a 7-day rota system that helps meet service needs, which requires working weekends on a rota basis; however, the schedule is set in advance to help you maintain a healthy work-life balance.

Responsibilities

* Visit people in their homes to assess social care needs and deliver person-centred plans.
* Assess daily living needs and develop personalised plans to regain skills and independence.
* Complete Care Act Assessments when ongoing services are required.
* Collaborate with a multidisciplinary assessment team and communicate effectively with service users and colleagues.
* Keep up to date with training and policy changes in Health and Social Care to ensure competent service delivery.


Qualifications

* Experience of working with people and clear, appropriate communication with the public.
* Ability to exercise judgement and prioritise workloads.
* Knowledge of current policy, practice and legislation within Health and Social Care.
* Willingness to undertake required training to perform the role effectively.
