Job Overview
A new and exciting opportunity has arisen for a skilled part‑time Health Care Support Worker (HCSW) to be part of the development of a redesigned Nursing Care Model with the Hillingdon Care Connection teams (CCT).
The CCT comprise a Guided Care Matron, GP, HCSWs, an administrator and well‑being and support assistants. Together they provide active case management for adult patients with long‑term conditions in order to prevent unnecessary hospital admissions.
The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and patients when working in our healthcare settings.
All applicants will need to be able to drive and have access to a car, as community staff travel considerable distances throughout the day.
Main Duties
* Home visits for clinical tasks
* Joint visits with matrons when required
* Observations and NEWS2
* Occasional medication monitoring – one week maximum
* Phlebotomy
* Simple wound care
* Eye drops – short term
* Supportive reviews
* Urinalysis
* Inhaler technique
* Pressure area checking
* Documentation
Following a robust induction you will be expected to work independently, escalating concerns in a timely manner to the Matron or GP who will have overall clinical responsibility.

Job Summary
Band 3 HCSW work closely with people who use our services, their families, friends and carers and are an important contributor to how people experience our services. They are expected to be kind and responsive but professional and informative and contribute to the quality of the services we provide by keeping the people using our services and working closely with Registered Nurses.
* Contributing to the best possible clinical outcomes by using up‑to‑date skills and adhering to evidence‑based policies and procedures when implementing care plans
* Ensuring the people using our services have a good experience by respecting, empowering and working in partnership with people when contributing to the implementation and evaluation of care plans

Person Specification
Essential Criteria
* Numeracy and literacy skills required to keep accurate computer records and administer medicines (if appropriate)
* Spoken and written English, to an appropriate standard to fulfil the job requirements
* Previous experience of working with a multidisciplinary health care team
* Experience of working with the general public and communicating with a range of different people
* Previous experience of working in a fast‑paced environment
* Previous experience of working within a customer‑focused environment
Desirable Criteria
* NVQ Level 3 or equivalent
* NHS or similar experience
* Formal caring role
Skills and Knowledge
Essential Criteria
* Able to communicate with the multi‑disciplinary team, update others in the team and use handover effectively, speaking face‑to‑face and by telephone
* Obtain consent and understand how the Mental Capacity Act and Deprivation of Liberty Safeguards impacts on your work and the people you are working with
* Able to maintain electronic patient records and use Microsoft Word to draft letters and reports
* Expected to adhere to the Code of Conduct for Health Care and Support Workers and Adult Social Care Workers in England
* Contribute to the implementation of care plans by helping to assess patients, develop, implement and evaluate a care plan
Desirable Criteria
* Basic knowledge and understanding of the Mental Health Act 1983
* Have undertaken the Care Certificate and further training as required
