To be responsible for theimplementation of processes for the effective management of patients withlong-term conditions using evidence-based practice including care for elderlyand housebound patients at the Practice.

Working as a senior member of the practicemultidisciplinary team, the post holder will ensure nursing services aredelivered effectively to the entitled patient population. The post holder willhave the opportunity to shape and develop within their position whilecontributing to the growth of the practice.

The Practice Nurse will be responsible for a numberof clinical areas such as infection prevention and control, health promotion, chronic disease management, healthpromotion, well women and well man clinics, as well as actively supporting thepractice management team in the reviewing and delivery of clinical policy andprocedure.


Main duties of the job

Toprovide a clinically effective practice nursing service to address the healthneeds of the practice population.

Toassess, plan and provide healthcare and nursing services to the practicepatients ensuring they are effective, relevant and of a high standard inproviding optimum levels of care. Services provided to include:

oManagementof patients with chronic diseases

oProvisionof contraceptive advice and assist GPs with contraceptive services.

oTakingblood and urine samples, other specimens, and swabs

oCarryingout Spirometry/Reversibility / ECG testing when required

oOfferingspecialist information and advice in areas such as blood pressure, weightcontrol and smoking cessation advice.

oSutureremoval / redressing following minor surgery performed in the practice.

oAdministeringtravel immunisations and offering travel healthcare advice

oProvidingadvice to patients in respect of their continuing medical and nursing needs

oAdministeringof gonadorelin analogue treatment, including provision of Zoladex and Prostap

oProvisionof ring pessary service


About us

Rochdale Health Alliance(RHA) was established in 2016, byGP practicesfrom across the RochdaleBorough, to streamline the way in which services are delivered and tocontribute to the development of the wider health and social care provisionacross the Borough. Heywood, Middleton and Rochdale Primary Care Academy ispart of RHA and develops the primary care workforce, in order to facilitate therecruitment and retention of well trained and motivated staff.


Job responsibilities

Towork towards achieving the targets laid down in the Quality and OutcomesFramework / Core Plus / IIF

To be involved in the development ofcare guidelines / protocols in collaboration with the GPs for use in practice

Referring to other members of thepractice team as necessary

To work in close collaboration withother primary care nurses, allied professionals, GPs, and practice staff toensure continuity of patient care.

Participate innational screening work such as cervical cytology and deliver the nationalimmunisation and vaccination schedule.

Act as achaperone when required.

Select, carryout and interpret results of specific pathology and other tests.

Carrying outseasonal clinics e.g., influenza

To assist other clinicians in the event of any emergency or resuscitationincidents.

To take the lead on Infection Prevention and Control carrying out 6monthly audits using national guidance.

Any otherduties as deemed appropriate by the Practice Manager.

Administration Duties

To maintain accurate patient records and enter onto the computer usingagreed templates and snomed codes.

Ensure accurate completion of all necessary documentation associated withpatient health care.

To collect data and participate in regular practice audit and research projects.

To maintain own annual registration with the NMC and adhere to Code ofProfessional Conduct

To ensure own revalidation is completed on time to continue smoothworking practice.

To check all Emergency Drugs Boxes on a weekly basis and ensure all itemsare replaced when expired / used.

To check emergency resus equipment on a weekly basis.

To carry out Infection Prevention and Control inspections monthly andproducing an IPC Audit on an annual basis and reporting on any infectioncontrol issues highlighted throughout the year.

To develop and review audit protocols and process for the practice,effectively utilising the audit cycle.

To develop, review and implement nursing protocols in conjunction withthe Directors

Participate in the review of significant and near-miss events applying astructured approach i.e., root cause analysis (RCA)

Support the practice team with the reviewing and implementation ofpractice policies and protocols, ensuring conformance to extant legislation.

Any otherduties as deemed appropriate by the Practice Manager or Nursing GP Lead.

In the performance of the duties outlined in this job description, thepost holder may have access to confidential information relating to patientsand their carers, practice staff and other healthcare workers. They may alsohave access to information relating to the practice as a business organisation.All such information from any source is to be regarded as strictlyconfidential.

Information relating to patients, carers, colleagues, other healthcareworkers or the business of the practice may only be divulged to authorisedpersons in accordance with the Practice policies and procedures relating toconfidentiality and the protection of personal and sensitive data.

The post holder is expected to adhere to practice policies and proceduresand relevant legislation including the requirements of any professional bodies.

Attend mandatory training as identified by RHA.

Employees must be aware of the responsibilities placed upon them underthe Health and Safety at Work Act (1974), to ensure that the agreed safetyprocedures are carried out to maintain a safe environment for employees andvisitors.

To always promote a professional approach by attire and attitude topatients and staff both in and out of the practice.

Attend and participate in staff and Practice meetings and in-houseclinical training sessions.

All members of staff have a responsibility to report all clinical and non-clinicalaccidents, incidents or near misses promptly and when requested to co-operatewith any investigations undertaken.

All members of staff have a responsibility to demonstrate due regard forsafeguarding and promoting the welfare of children.

Person Specification


Qualifications

* Registered Nurse (Nursing and Midwifery Council)
* Mentor or teaching qualification
* Post graduate diploma or degree (Chronic Disease Management)
* Nurse prescriber
* Family planning qualification


Experience

* Experience of working in a primary care environment
* Experience of chronic disease management ie Diabetes, Asthma, COPD, CVD, CDs
* Experience of working autonomously
* Experience of working as a practice nurse or community nurse
* Current cervical cytology taker
* Child and adult immuniser
